How to make a claim
DUTY OF DISCLOSURE

Before you submit your claim, please be aware that you will need to disclose to Us anything that you could reasonably be expected to know is relevant to Our decision to accept the risk of insuring You.

The duty of disclosure applies to you and everyone insured under the contract of insurance, on behalf of you. If you, or they, fail to comply with the duty of disclosure, We may reduce Our liability under the contract in respect of a claim or may cancel the Policy. If the non-disclosure is fraudulent, We may treat the Policy as if it never existed and pay nothing.

Please be advised of the following requirements when:

Your phone has been Lost or Stolen:

1) Please notify One NZ immediately to bar your SIM card. You may also want to ask One NZ to stop any data usage if your mobile has this capability.
2) You will need to obtain a police report before (submitting) lodging this online claim. If your mobile phone was lost or stolen overseas and an overseas police report was not obtained, you will need to complete a statutory declaration.

Your mobile phone has been Damaged:

1) After you have lodged your online claim you will need to take the damaged phone to a One NZ dealer where the damage assessment form can be completed.
2) After you have lodged your online claim, you must surrender your damaged handset to the One NZ dealer, unless otherwise arranged through phoneInsure.

Other Key Information:

1) Please ensure that your account is paid to date.
2) Allow 48 business hours to be contacted.
3) On claim approval, delivery of a replacement handset will be 1-5 business days.
4) The replacement mobile phone with Sim Card (if required) will be couriered to you as a signature only package. Therefore we recommend the delivery address be either your daytime work address or your nearest One NZ outlet.
5) If claiming for accessories, proof of purchase is required.
Note: you do have the option of calling the One NZ phoneInsure claims call centre during business hours on 0800 505 021 to lodge a claim. You cannot lodge a claim in a One NZ store.
